What Are Medicare Lifetime Reserve Days?
Lifetime reserve days are 60 extra days of inpatient hospital coverage that Medicare Part A provides if a single hospital stay runs longer than 90 days. They act as a one-time backup — not something you get every year.
Key takeaway: You get a total of 60 lifetime reserve days for your entire life — not per year, and not per hospital stay. Once a reserve day is used, it’s gone for good.
How Lifetime Reserve Days Work
A handful of rules govern how and when you can use these days. Here’s what matters most.
- ✓ Standard limit: Medicare Part A fully or partially covers the first 90 days of an inpatient hospital stay in a benefit period.
- ✓ Extra coverage: Days 91 through 150 use your lifetime reserve days, if you choose to use them.
- ✓ No renewal: You get 60 of these days for your entire life. They do not reset or renew each year.
- ✓ Splitting days: You don’t have to use all 60 at once. You can use a few during one long stay and save the rest for a future stay.
- ✓ Saving your days: You can tell the hospital you don’t want to use your lifetime reserve days, so you can pay out-of-pocket instead and save them for later.
- ✓ Where they don’t apply: Lifetime reserve days are for inpatient hospital stays only — they don’t apply to skilled nursing facility stays.
2026 Costs & Coinsurance
Here’s what a single inpatient hospital stay costs at each stage, under Original Medicare alone, in 2026.
| Stage of Your Stay |
What You Pay |
| Days 1–60 |
One-time $1,736 deductible, then $0/day |
| Days 61–90 |

100% of all hospital costs |
Figures reflect 2026 Medicare Part A cost-sharing amounts and are subject to change annually.
Example: A 110-Day Hospital Stay
Numbers make this easier to picture. Here’s how a single, continuous 110-day inpatient stay would play out under Original Medicare alone.
| Stay Period |
What You Pay |
Running Total |
| Days 1–60 |
One-time $1,736 deductible |
$1,736 |
| Days 61–90 (30 days) |
$434/day coinsurance |
$14,756 |
| Days 91–110 (20 lifetime reserve days used) |
$868/day coinsurance |
$32,116 |
At discharge, this person has used 20 of their 60 lifetime reserve days — leaving 40 still available for any future stay that runs past 90 days in a benefit period. How a Medigap Plan Changes This
Standard Plan G and High-Deductible Plan G cover the exact same benefits — including lifetime reserve day coinsurance. The difference is when that coverage kicks in.
|
Standard Plan G |
High-Deductible Plan G |
| 2026 Annual Deductible |
$283 (Part B deductible only) |
$2,950 |
| Lifetime Reserve Day Coinsurance |
Covered 100% after your $283 deductible is met |
Covered 100% once you’ve spent $2,950 out-of-pocket for the year |
| Monthly Premium |
Higher |
Often 60–70% lower |
In the 110-day example above, standard Plan G would bring your out-of-pocket cost down to essentially $0 after your $283 deductible. With High-Deductible Plan G, you’d cover your own costs until you reach $2,950 for the year, after which the plan pays 100% of what’s left.
Frequently Asked Questions
A few of the questions we hear most often about lifetime reserve days.
Do lifetime reserve days renew every year?
No. You get 60 lifetime reserve days total, for your entire life. Once you use one, it does not come back.
Can I save some of my lifetime reserve days for later?
Yes. You can use a portion of your 60 days during one hospital stay and save the rest for a future stay. You can also decline to use them at all during a stay and pay out-of-pocket instead, preserving your full bank of reserve days.
Do lifetime reserve days apply to skilled nursing facility stays?
No. Lifetime reserve days only apply to inpatient hospital stays. Skilled nursing facility coverage follows a separate set of rules and day limits.
What happens after I’ve used all 60 lifetime reserve days?
If a hospital stay continues past day 150 in a benefit period and you have no lifetime reserve days left, you are responsible for 100% of the hospital costs from that point forward — unless you have a Medicare Supplement or other coverage that helps.
