The Gap Medicare Advantage Doesn’t Talk About
Most Medicare Advantage plans on the Treasure Coast and Space Coast advertise low or $0 monthly premiums. What they don’t put on a billboard is the cost-sharing built into the plan: a daily copay for every day you’re hospitalized, an ER copay whether or not you’re admitted, and out-of-pocket maximums that can run into the thousands of dollars in a single bad year.
A hospital indemnity plan doesn’t replace your Medicare Advantage coverage. It works alongside it, paying you a cash benefit — not the hospital, not the insurance company, you — when a covered event happens. You decide how the money gets used: a copay, a prescription, rent, gas for the drive to a specialist. That’s the entire idea behind GTL’s Indemnity Plus Elite.
Introducing Indemnity Plus Elite from GTL
Guarantee Trust Life Insurance Company (GTL) has underwritten health and life insurance since 1936. In 2005, GTL introduced the first hospital indemnity product built specifically to help Medicare Advantage members cover their out-of-pocket costs. Indemnity Plus Elite is the current, most comprehensive version of that plan.
Coverage is available through Select Benefit Services Association (SBSA) membership, which also includes a set of everyday discounts — fitness clubs, dental and vision savings programs, and more — described further down this page.

Step 1: Choose Your Base Hospital Confinement Benefit
The base plan pays you a daily cash benefit for each day you’re hospitalized, including a 24-hour observation stay. Pick the benefit period and daily amount that fits your budget:
| Base Plan Choice |
Daily Benefit Amount |
| 1-Day Benefit Period |
Up to $2,500/day |
| 3, 4, 5, 6, 7, 8, 9, 10, or 15-Day Benefit Period |
Up to $990/day |
Whichever plan you choose, a $15/day Basic Daily Benefit continues for the remainder of the 31-day maximum benefit period, and your full benefit restores an unlimited number of times after 60 days with no hospital confinement.
Automatically Included With Your Base Plan
Short Duration Hospital Stay Benefit — If you’re hospitalized and discharged between 6 and 24 hours, for either observation or confinement, you still receive your chosen daily benefit (25% of the daily amount under the 1-Day base plan).
Emergency Room Benefit — Pays $150 for a covered ER, emergency care, or urgent care visit due to an accident or injury. No hospital admission is required to trigger this benefit.
Mental Health Benefit — Pays $175/day for up to 7 days if you’re hospital-confined for a mental or nervous disorder. (Paid in lieu of, not in addition to, the standard hospital confinement benefit.)
Step 2: Add Optional Riders to Fit Your Needs
Every client’s risk and budget looks different. These riders let you build coverage around what worries you most — cancer, a fall, a nursing home stay after a hospitalization, or day-to-day dental and vision costs.
Skilled Nursing Facility Rider
Pays $100–$300/day after 3 consecutive hospital days, if SNF admission occurs within 30 days. Choose days 1–50 or days 21–100 of coverage.
|
Cancer Lump Sum Rider
A cash payout of $2,500–$20,000 on a first cancer diagnosis (25% for Cancer In Situ, $500 for basal/squamous skin cancer). A 30-day waiting period applies. An optional Recurrence Rider restores the benefit at 10%–100% based on years in remission.
|
Critical Accident Rider
Lump sum cash for ER-treated accident injuries under a $5,000 or $10,000 plan — hip/skull fracture, hip dislocation, knee injuries, and an accidental death benefit. 30-day waiting period applies.
|
Ambulance Rider
Pays $50–$400 per ground or air ambulance ride, up to 4 times a year (lifetime max 12 trips). No hospital confinement required.
|
Outpatient Therapy Rider
$50/day for physical, occupational, or speech therapy (15 or 30 days/year max). Chiropractic care covered at $50/day, up to 5 days/year, for accident-related injury.
|
Outpatient Surgical Rider
Pays $250–$1,000 for a surgical procedure done in an ambulatory surgical center or hospital outpatient facility, up to twice a year.
|
Lump Sum Hospital Rider
An extra $250, $500, or $750 lump sum per confinement period, on top of your daily benefit, to help offset a plan’s hospital copay. Restores after 60 days claim-free.
|
Dental & Vision Rider
Up to $400, $800, or $1,200 a year (after year one) toward dentist, ophthalmologist, or optometrist visits, including $200 toward eyeglasses or contacts. 3-month waiting period.

Step 3: The Benefit Is Yours to Use However You Need
When you have a covered claim, GTL pays the benefit directly to you (or your assigned designee) — not to the hospital, not to your Medicare Advantage carrier. There’s no restriction on how the money is spent. Many clients use it for hospital copays; others put it toward everyday bills while they recover.
Claims are simplified and underwriting is streamlined, with instant approval available and guaranteed issue for ages 64½ through 70.
Is This Plan Right for You?
Indemnity Plus Elite is designed to work alongside a Medicare Advantage plan, filling in the daily hospital copays, ER costs, and out-of-pocket surprises that a $0-premium plan doesn’t cover. It’s a strong fit if you:
- Are enrolled in — or considering — a Medicare Advantage plan and want a cushion against hospital and ER cost-sharing
- Want cash benefits paid directly to you, with no restriction on how they’re used
- Are between ages 64½ and 70 and want guaranteed-issue, no-medical-exam approval
- Have a family history of cancer or accidents and want an added layer of lump-sum protection
Important: This is a supplement to health insurance, not a substitute for major medical coverage. Indemnity Plus Elite is not a Medicare Supplement policy and does not fully supplement Medicare. Coverage is issued by Guarantee Trust Life Insurance Company on Certificate Form GC2150-IL and associated riders, and is subject to exclusions, limitations, waiting periods, and pre-existing condition provisions. Membership with Select Benefit Services Association (SBSA) is required to apply. This page is a summary — for full costs and details of coverage, please request your personalized quote below.
